If you wish to sign up for our 10 Week Program, you could be required to submit your name, email, address, phone contact and payment information and you may also be asked to submit additional information including, but not limited to, your physical characteristics and your health goals.
Please be aware that all payment information will be shared with and stored by our third-party payment processors, Stripe.
